POSITION SUMMARY:
The Infant & Child Wellness Worker is responsible for the delivery of programs and services that aim to strengthen family wellness starting prenatally, as well as throughout infancy and toddlerhood for families in the community.
The Infant & Child Wellness Program is an integral part of the Prevention program. The overall purpose of the Prevention Program is to prevent child welfare involvement in families and to reduce the number of children in care. The Infant & Child Wellness Program plays a key role in preventing child welfare involvement in families by 1) providing support, education and awareness to families starting at the very beginning of a child’s life and 2) helping parents/caregivers to support their children throughout the most critical developmental period of their lives. However, in the case that families are already involved with child welfare, the Infant & Child Wellness Program collaborates with parents/caregivers to address and resolve the identified child welfare concerns.
The Infant & Child Wellness program provides both one-to-one support for families and group/community programming. The role of the Infant & Child Wellness program is to provide 1) one-to-one in-home support to parents/caregivers, and 2) provide community programming/activities relevant to promoting family wellness of this specific population. The length of time that a family stay connected to an Infant & Child Wellness Worker is dependent on the needs of the family, as well as their family wellness goals.
RESPONSIBILITIES:
Receive referrals, connect with family, build rapport, develop Family Wellness Plan. Assist family in achieving wellness goals through in-home support services (cleaning, organizing, meal planning, routines, budgeting, educational awareness, etc.), facilitating tailored therapeutic activities, connecting them to appropriate programming/referral sources and providing financial assistance when necessary (baby essential needs pantry, Support Services budget). Conduct Ages & Stages Screener (Nurturing the Seed) when necessary and assist in Developmental Support Planning with family. Deliver consistent community programming with a focus on parenting/parent-child relationship (I.e., Making the Connection, Calm Cool Connected, Healthy Family Kitchen, Talk & Task, pre/post-natal vouchers, etc.) Facilitate educational/awareness presentations on the following topics: infant/early childhood mental health, nutrition, screen time, attachment, the importance of routines, maternal/paternal mental health,
family bonding, etc. Collaborate with local community resources on various initiatives that promote family wellness in your target population when available (NAAW, Mental Health Week, etc.)
