Position:        Prevention Intake Worker                             Term:              Full Time/Contract

Department:  Community Safety & Well-Being                   Salary Range: TBD

Hours:            35 Hours per week                                          Location:       Fort Albany, ON

SUMMARY:

The Prevention Intake Worker will be responsible for receiving all new prevention referrals, ensuring that they are completed accordingly and sent to the appropriate supervisor for review. The Prevention Intake Worker will also be responsible for assisting all clients located in Fort Albany with Crisis Funding applications. The Prevention Intake Worker will also assist their team with coordinating, planning, and delivering services and programming. Assist other team members with facilitating referrals to other service providers or treatment programs as necessary for the accomplishment of the treatment plan. Provide written reports in accordance with the Prevention program’s policies and procedures by reviewing client referrals, collateral reports, and meeting with clients. Maintain accurate case notes written within twenty-four hours of contact regarding parenting counseling sessions, collateral, and case conference information.

 

RESPONSIBILITIES:

·         Receive referrals, connect with family, build rapport, develop Family Wellness Plan.

·         Assist family in achieving wellness goals through in-home support services (cleaning, organizing, meal planning, routines, budgeting, educational awareness, etc.).

·         Facilitating tailored therapeutic activities, connecting them to appropriate programming/referral sources and providing financial assistance when necessary (baby essential needs pantry, Support Services).

·         Deliver consistent community programming with a focus on parenting/parent-child relationship.

·         Facilitate educational/awareness presentations on the following topics: infant/early childhood mental health, nutrition, screen time, attachment, the importance of routines, maternal/paternal mental health, family bonding, etc.

·         Collaborate with local community resources on various initiatives that promote family wellness in your target population when available.

 

QUALIFICATIONS:

·         Bachelor of Social Work Degree or relevant Social Services Diploma.

·         Minimum Two (2) years in the Social Service Work field.

·         Possess a valid Driver’s Licence with a clean Driver’s Abstract.

·         Must provide a Vulnerable Sector Check.

·         Must be willing and able to travel.

·         Must be available to work flexible hours.
